Important Notes to Candidates Applying for KUDDE Courses by Downloading Application Form from Website

1. Download all the forms.
   a) Main Application Form, i.e., OMR Sheet (double sided)
   b) Examination Application Form & Hall Ticket Forms (2 Separate Pages)
   c) Registration Card (duplet and double sided)
   d) Bank Chelan Form
   e) Address Slips (for communication)
   f) MBA Entrance Test Application Form.

2. MBA Entrance Test Application Form is optional - only for those applying for MBA Programme.

3. Get all the forms printed in colour on good quality (100 GSM) A4 size Paper.

4. Without fail, refer Courses Offered and Instructions to fill OMR Application Form sections before filling application forms, and follow the instructions given there.

Important: Candidates applying by downloading application forms from the website should add Rupees 150/- (towards application form fee) to the Course Fee and pay it through a Bank using the Challan Form or obtain a Demand Draft (payable to Finance Officer, Kuvempu University). The DDE part of the Challan or the DD should be sent along with the application form. Refer General instructions to the Candidates section.
