One attested copy of marks card of Final year
examination & passing certificate of qualifying examination
alongwith original marks card (i.e., B.A., B.Sc., B.Com.)
issued by the University for verification & return. PUC or
10+2 or equivalent for U.G. Courses. |

Original marks cards of SSLC, PUC and Degree
for verification and return, and one set photocopies of the
same. |

6. |
Examination Application Form and Hall Ticket
form - affix one photograph in the space provided for it. Some
of the boxes in this form require just "P"

Admission Registration (Confirmation) Card
along with stamp size photos wherever required. Photos affixed
on this card should be attested by a Gazetted Officer. |

Correct Address for correspondence is to be
written or typed neatly and legibly by the candidate on all
the six address slips provided and enclosed along with the
application at the time of submission. |

The Admission, Examination application formats can also be

Candidates who use the downloaded format should add Rs. 150/-
extra along with their fees for admission towards the
application fees. |

Important INSTRUCTIONS |
| |
|
|
1. |
After admission is confirmed, an acknowledgment
card is issued. Candidates must quote the Enrollment /
Registration Number, Course and the Exam, for which
registration is sought. In addition to the above in any
further correspondence for exam or any academic purpose the
Admission Number allotted should be mentioned. |

2. |
Application for admission registration should
be submitted to The Director, Directorate of Distance
Education, Kuvempu University, along with the necessary
certificates, on or before the last date fixed for the
purpose. (see Academic Calendar of Events) The penal fee will
be paid along with the registration fee. The fees to be paid
should be in the form of Challan of Electronic Transfer (See
details at Para 6 below or Para 2 of General Instructions).
The Challan should be enclosed along with the application form
as stated above. |

3. |
The prospectus for any concerned admission
during a particular year including the syllabus and the
application form shall be valid for the said year only. |

The candidates should send their Application
for Registration by Registered Post with Acknowledge Due/Speed
Post. (Please do not fold the Application Form). |

|
5. |
Electronic Fee Transfer :
The University has introduced the Electronic
Transfer of Fees to facilitate the smooth functioning and
maintaining of records of fee paid by the student. The
student is advised to utilise the two sets of challans in
quadruplet (perforated) enclosed with the prospectus for the
purpose. If the student is paying the entire fee in a single
installment, he/she may utilise only one challan form. If the
student is paying in two installments, he/ she may use one
challan for the first installment of fees and the second
challan should be retained to be utilised for the payment of
the second installment of fees. |

Procedure: The
procedure for payment is totally simplified. The candidate is
advised to fill in the details of the subject beside the list
of courses printed in the challan with details of fee paid,
installment if any, and submitted to any State Bank of Mysore
or State Bank of India or any Nationalised Bank branches
anywhere in the country. The challan in quadruplet will have
to be submitted to any of the above banks with the fees. One
counterfoil should be retained by the candidate, one should be
attached to the application to be submitted to KUDDE.
Candidates should note that they NEED NOT fill in either the
account number or bank code which is already printed in the
challan. |

fees in a single installment the facilities for two
installments is also provided. if the second installment is
not paid in time, such students shall not be permitted to take
the exam and the first installment will be forfeited. |

Note :
The Directorate of Distance Education reserves

adequate number of students are not available for any course.
If students have paid admission fees, such fees will be
refunded to the students. |

SPECIAL NOTE
Candidates applying through the Student Counseling Centres
should read the following instructions carefully regarding the
duties and functions of the Kuvempu University Directorate of
Distance Education and Student Counseling Centres.
Candidates can submit their
filled in applications at the Student Counseling Centres with
original certificates and Challan/ DD. They should take an
acknowledgement from the Student Counseling Centres for the
same.
They should collect their originals from the Student
Counseling Centre immediately after the Admission is approved
by the Directorate of Distance Education.
The Student Counseling Centre shall be totally responsible for
collection and return of the original certificates of students
who have submitted their applications through them. The
Directorate of Distance Education will not be responsible for
the above transaction and the Student Counseling Centres will
be solely responsible for the above.
The
Student Counseling Centres shall arrange for conducting
Orientation/Counselling and Practicals for prescribed number
of days with prior approval of Kuvempu University Directorate
of Distance Education.
The Student Counseling Centre
shall arrange for sending the Practical Records for Science
students duly signed by the Competent person/Student
Counseling Centre Co-ordinator/ Examiners.
The Student Counseling Centre
shall provide all necessary information regarding Admission,
Course details and such other information provided by the
University to the students.
The Student Counseling Centre
shall not misguide the students with regard to Admissions,
Course details, Examination, Term of the courses concerned and
such other aspects.
The University does not offer
any course on one sitting basis. The Student Counseling Centre
shall not announce/advertise or provide mis-information to the
students on this matter and shall not give any false assurance
to the students such as for example, "Pass guarantee," etc.
If any Student Counseling Centre
indulges in such practices, the students may immediately
contact the Directorate of Distance Education for
clarification.
The University shall not be
responsible for any involvement by the students in the above
said matters with the Student Counseling Centres.
The students shall procure a
single Challan/ DD for the total amount shown in the Fee
structure for the respective course and enclose the same along
with the filledin Application and submit it to the Student
Counseling Centre. (For Fee and Challan/ DD details see 'Fee
Structure' and 'Instructions to the Candidates' in the
prospectus. Also see - procedure for Admission through Student
Counseling Centres).
However provision for payments
of fees in two installments is also made for those students
who are not able to pay the full fees at one time.
After the approval of
Admissions, the Directorate of Distance Education shall send
Admission Registration Card directly to the students.
The Directorate of Distance
Education shall directly / or through the Student Counseling
Centre send the Syllabus, Study Material. Internal Assessment
Topics with further instructions to the students.
The Directorate of Distance
Education shall send the Annual Examination Time table,
Admission Ticket and allotment of Examination Centres
directly. If students do not receive it, they should
immediately contact the DDE office or their Student Counseling
Centres.
Results and Marks card will be
sent directly to the students / or through the Student
Counseling Centre. The copy of results will also be available
from the Student Counseling Centres. |

The Kuvempu
University Directorate of Distance Education can authorize the
Student Counseling Centre to conduct the Practical Courses or
make alternative arrangements. The Kuvempu University
Directorate of Distance Education may merge more than one
Student Counseling Centre to a designated Examination Centre.
Hence, the candidates should note that all Student
Counseling Centres may not be Theory Examination Centres.
However, the University will make efforts to designate
examination centres within reasonable distance from the place
of residence. The University may also consider conducting
exams continuously without a break or gap between exams. |
