INSTRUCTIONS TO FILL OMR/ICR APPLICATIONS

 

A computerized OCR/OMR scanner will process your application form. Therefore, it is very important to follow the instructions carefully. In case the form is damaged or the instructions for completing the form are not followed, the form is liable to be rejected.

USE ONLY BLACK INK BALL POINT PEN TO FILL UP THE APPLICATION FORM

   

DO NOT FOLD OR ROLL THE FORM

   

DO NOT STAPLE OR PIN ANY CERTIFICATES TO THE FORM

   

HANDLE THE APPLICATION FORM WITH UTMOST CARE

Note 1 : The Candidates should carefully go through the enclosed OMR application form that is given with the Prospectus. Also, the Candidates are advised to get a photocopy of the OMR application form and fill it up, as instructed, before filling up the original OMR application form. This will avoid errors in filling the original form.

Note 2 : Candidates are also advised to use only standard letter styles, i.e., Fonts while filling the OMR Form. Do not fill it in a hurry. Your written alphabets and numerals should closely resemble the alphabets and numerals shown below.

The OMR form consists of items 1 to 24. Every box in each item is to be filled in BLOCK LETTERS ONLY.

Item 1 - Name of the Candidate : The name should be spelt exactly as it is printed on your SSLC/Degree Certificate/ Marks Card. Fill your name in capital letters in the boxes. Leave one blank box between two words. In case your name will not fit in the boxes provided, shorten it appropriately. For e.g. if your name is Panduranga Madhavarao Narayana Das then you can shorten it as PANDURANGA MADHAVARAO N D

Item 2 - Name of father: The name of father/ guardian should be written as it is printed in your S.S.L.C. Marks card.

Item 3
- Sex Code: In this box, write 0 for Male and 1 for Female

Item 4 - Category Code: Candidates should mention 1 for SC, 2 for ST, 3 for OBC and 4 for GM.

Item 5 - In this box write 1 if you are Physically Handicapped, 2 if you are Visually Handicapped. If neither, write 0.

Item 6 - Medium: Candidates should fill the medium of instructions in which he/ she has to write the examination.

Item 7 - Nationality: In this box, write 1 for Indian citizen and 2 for Other nationality

Item 8 – Student Counseling Centre Code: Candidate should write the Student Counseling Centre Code of the respective Student Counseling Centres as per the list given in the Prospects. Eg: 01 for KUDDE, Shankaraghatta. Note: Student Counseling Centre means where admission is registered.

Item 9 – Student Counseling Centre Name: In this box, write your Student Counseling Centres Centre name. List of Student Counseling Centres names and code are given in a separate page at the end of the prospectus.

Item 10 - Date of Birth: Candidate should write Date of Birth as is printed in the S.S.L.C. marks card.
Eg: 01-10-1980

Item 11 - Subject Code: The candidates should write the Course Code in the box provided as mentioned in the Prospectus. Eg: for M.A. Kannada, PGA01, for B.Com. UGC31.

Item 12 - Course: The Candidates should write the course name like B.A., B.Com., M.A., M.Sc., M.Com., PGD etc.

Item 13 - Subject: The Candidate should write the Subject name like Kannada, History & Archaeology, Physics etc.

Item 14 - Occupation: Candidate should write his/ her occupation / nature of work. Eg: Agriculturist, Teacher, Professional etc.

Item 15 - Annual Income: Candidate should write his/ her or guardian’s annual income.

Item 16 - Languages: In these boxes, only I & II year B.A, B.Com. & B.Sc., and I year BBM candidates need to write their choice of two languages, among them one should be English. Language codes: Kannada - 1, English - 2, Hindi - 3, Sanskrit - 4 and Urdu - 5.

Item 17 - Admission Registration / Examination Fees Paid: The Fees should be paid by Demand Draft/ The DD/ or by Bank Challan, Amount, Number, Issued date & Bank name need to be written in the appropriate boxes provided.

Item 18, 19 & 20 - Complete permanent postal address of the candidate in CAPITAL BLOCK letters, Cell Phone/ Telephone No. with STD Code, E-mail. The candidate should write his/ her complete postal address in block letters in the space provided. Do not write the name here. Write the PIN code and telephone number in the boxes. The candidates should write the postal address in short. PIN code is compulsory. Candidates need to write their E-mail-ID for future correspondence.

Item 21 & 22 - Institution / University from which the qualifying examination is passed, Qualifying Examination, if any: The Candidates should write month & year of passing of their graduate degree / pre-university in the given box along with subjects taken and class obtained.

Item 23 - List of Papers with Paper Code: Paper Code & Title of the Paper: Write the Paper Code and Title of the Papers as mentioned in the Prospectus.

Item 24 - Exam to Appear: Candidates should fill the box if he / she is appearing for the First/ Lateral Exam.

Lastly, do not forget to Put your Signature.

Important Note:

Application Number: It is already bar coded, do not scribble or write anything in that space.

Affix your recent stamp sized coloured photograph in the space provided using good quality glue/gum. The size of the photograph should not exceed the size of the box printed. Do not pin or staple. Affix your signature in the space provided below the photograph.

Read carefully the declaration printed on the reverse of the OMR Application Form and sign, failing which the application will be rejected.

Address Sticker/Admission Registration Card

Candidates have to carefully fill in the Address Sticker and Admission Registration cards, and send it alongwith the Application form to the Director, DDE, Kuvempu University. Acknowledgement of receipt of application may be obtained in person or by post.
